Step 1: Apply and Request Your Preferred Date & Time
Click the button above to select a time slot for your initial evaluation. You will be prompted to answer a brief set of screening questions so we can understand your primary goals and ensure our care model is a strong clinical fit.
Step 2: Application Review (Within 24 Hours)
To submit your request, you will enter a credit card to hold the appointment. Your card will NOT be charged yet. Our team will review your screening answers within 24 hours to confirm clinical alignment and state residency.
Step 3: Confirmation & Intake
Once your request is approved, your appointment is locked in, and the initial evaluation fee is processed. You will instantly receive a secure email with your comprehensive clinical intake paperwork to complete before we meet.
